What is Hospice?
Hospice is a philosophy of care that has been in place since the Middle Ages. Hospice provides care and support to terminally ill patients and their families. This care and support continues beyond the death of the patient with bereavement services for families and friends.
Hospice is not less care, but different care. The focus of hospice is not on the disease, but on the symptoms. The dignity of the patient and the patients quality of life are of paramount importance.
The Hospice Team
The Hospice Team is made up of people from a variety of disciplines who meet the different needs of the patient. Physicians, nurses, home health aides, social workers, trained volunteers, clergy and therapists all combine their skills to ensure the best service to the patient and the patients family.
Under the direction of your physician, a plan of care dealing with the medical, social, emotional and spiritual needs are implemented by the Hospice Team. Included in this plan are issues that affect the family, such as bereavement counseling.
A Patient is Eligible for Hospice if:

The patient, of any age, is in the stages of an advanced disease where the physician and medical director concur that there is a limited life expectancy.

A patient is under the care of a physician.

Patients, family or significant others desire and agree to receive services from Hospice and sign a consent form.

The patient has a primary caregiver in the home, or plan for care when they can no longer be alone.
Who Pays for Hospice Services?

Medicare and Medicaid, as well as many private insurance companies, include Hospice care as a benefit.

Hospice of Salem County will provide care if needed, regardless of insurance coverage or the ability to pay.
What About Bereavement Counseling?
Hospice of Salem County offers a variety of services that can help families and caregivers through their period of grief.
What Services Does Hospice Provide?
Nursing Care - Hospice nurses provide for the medical needs of the patient, including pain management.
Home Health Aide - Certified Home Health Aides provide for personal hygiene needs, meal preparation, and maintaining a healthy and safe environment.
Social Worker - The Hospice Social Worker is a person qualified to help the patient and family deal with their emotional, psychological and social needs.
Pastoral Care - Spiritual services are provided by qualified team members who coordinate their efforts with local clergy or spiritual counselors.
Volunteers - A Hospice Volunteer is assigned to each patient and their family to provide companionship, respite and friendship.
Additional Services - The Hospice Program provides physical, occupational, respiratory and speech therapies and nutritional counseling to improve the quality of life for the patient.
Medical Equipment - The Hospice Program can arrange to have the necessary medical equipment and supplies delivered to your home.
Hospice of Salem County was founded in 1987. It has served terminally ill patients and their families in the Salem County area with care and compassion. Hospice of Salem County serves Salem and parts of Gloucester and Cumberland Counties. Hospice of Salem County is dedicated to the belief that it is "a family of caring people, caring for families in the community."
