The Case Management Department of The Memorial Hospital of Salem County is open Monday through Friday from 8:00 a.m. to 4:30 p.m.

Case Managers assist physicians, nurses, patients and families with the discharge planning needs of all Hospital inpatients. In addition, the Case Managers are available to assist with discharge planning needs from the Emergency Room. Services provided include nursing home placement, post acute care rehabilitation referral, crisis counseling for families of admitted patients and more.
